Pretty much what everyone is saying here. Just want to add something that always works for me and minimizes the stress or the point of having to ask "how to escalate" and that is: The girl has already decided way in advance, probably the day she agreed with you on the movie date, to fuck you. The hardest thing to remember is to pull back. Don't just push.

A girl might want you sexually, but that all fades when the discomfort becomes noticeable to her. I don't care if it's brad pitt, or whatever hot guy. If he just comes at her like an aggressive rapist, she's going to be too uncomfortable to consent.
