Any non-neccessary, non-therapeutic proceedure resulting in any permanent effect performed on an individual incapable of consent to or even of interest in said proceedure is wrong and ought to be outlawed. The reasons why such a proceedure is done are entirely irrelevant as are the specific nature, severity, or degree of risk to the person undergoing the proceedure. The threshhold of unacceptability is reached before the details of any given proceedure of this sort even need to be addressed.

He's coming at it from the wrong direction. The basic questions are:

a) was it medically necessary?

b) was it consented to by a competent, majority aged patient?

Are all you need. If both are 'no', that surgery shouldn't happen.

I cannot say with complete certainty that they are morally equivalent and I cannot think of a reason that they are not equivalent. Maybe asking if they are morally equal would be better as that does not have as strict a mathematical definition.
