A woman chained up in a basement and forced to breed children for 20 years also isn't dying. Does that mean her needs were being met simply because she didn't die? I'd hope not.

Obviously, 'that which is required to survive' is a poor definition of 'need'. 'Need' should be defined as something more along the lines of 'that which is required to live a healthy life', which would rule out things like forced breedings over the course of 20 years or prolonged sexlessness.

Or, perhaps you do think such a woman's needs would indeed be being met. In which case, I have to congratulate you on your consistency!
