Okay, so this was a pretty short window of time, and kind of a lot to take in. For context, this whole scenario played out in about sixty seconds (a full minute).

I'm walking to the men's room in our building, which is just outside our office. I walk out, and happen to see a woman carrying a bunch of boxes, trying to open a glass door in the office directly across from mine. She's clearly struggling, and looks like she's trying to find a place to set her boxes down, so I jog over real quick and hold the door for her.

She looks over, super happy and says "that's so sweet, thank you!". I only then notice she's actually pretty hot. She said something about struggling with all the boxes, I just say no problem, happy to help.

We walk over to the elevators (same direction as the men's room), and an older coworker of mine is also leaving at the same time. He jokes I just came over to help because she was carrying a bunch of boxes of alcohol, she laughs and says they were celebrating Cinco de Mayo. She thanks me several times, saying how 'sweet' I was, the elevator arrives, I wish her a happy Cinco de Mayo, and head to the men's room while they step on the elevator.

Immediately after I step in the men's room, it dawns on me; she was, like, super thankful I'd gotten the door for her. Smiling, kind of intense eye-contact, even as I walked away and she got on the elevator, she was still smiling at me.

That's when it hit me:

I am a fucking IDIOT. I totally should've made a move, right?

Maybe have kind of lingered and struck up conversation? Offered to help carry some of the boxes for her? Just ask what her plans were for the actual day of Cinco de Mayo?

Clearly, I'm not good at just being "in the moment". I'd like you guys' input on how I should have handled that situation, though, just so I can learn and do better next time I'm in a similar situation.

Bonus question: if I happen to see her around again (I don't actually know if she even works in the building, or was just visiting), should I maybe make a move then? Or do you kind of have to make something happen in the moment?
