It’s based on my observations from talking to a lot of guys who call themselves incels. None of them ever cited “recovering from surgery” as one of their problems. They usually think they’re too short, too ugly, or their nose is wrong or their dicks are too small but I’ve NEVER seen someone unironically identify as an incel because they had to recover from a surgery. That’s just not representative of the online incel community.
