If you're going to approach during the day, I think direct is better. If you just have an aimless conversation it can come off as weird and she is left wondering what your intentions are. At least if you open directly ("Excuse me, this is really random but I thought you looked cute and I wanted to say hi") she knows immediately why you are there. Your first impression is that you are straight to the point, confident, and 100% authentic. That's never a bad image to put up.

I had the exact same mental blocker. The easiest way to get past it is through positive reference experiences. I'm not over my fear of rejection, but it is a fear. Like any other fear (public speaking, heights, etc) the way past it is through voluntary exposure. Go out and get rejected so often that it doesn't bother you anymore. Don't worry about your pseudo "perfect record". A man who has never failed is a man who has never tried anything new.

I stand up for myself when I need to. Don't get me wrong- I'm not always nice. If someone messes with me or the people I love, I will not tolerate it. But that's exactly the issue - I never have to show that side of me to someone I just met, right? In a similar fashion, I am very dominant and sexual ONCE I KNOW SHE IS INTO ME. But in order to get women interested, it is sometimes necessary to be dominant and sexual. In a sense it feels very circular.
