Even if we assume her nose got broken and still looks fine from straight view, but that would also mean he headbutted her straight. There is no way a headbutt would also hit the eye, and makes an even color black eye. When you get a black eye, the center of the hit mostly gets much worse. Here is definitely a makeup.
