No there are multiple different points on a timeline. After the first timeline in my state you can only assert fraud, DNA, etc. Do you practice law?

I think you can still get out of it by submitting to a DNA test but I don't practice in those jurisdictions so I can't say for certain.

You have three years after being listed on the birth certificate in most states to adjudicate if that is your kid or not. Even if they can find the real dad. Sucks.

They are broad, one of them asks something like: Is there anything in your past which might reflect poorly on your moral character? Men and women have real differences in medical costs for various reasons and should not be forced to be treated equal in matters where they are not in fact equal. This would be similar to forcing a 300 pound person to pay for food insurance at the same rate as a 100 pound person.
