It's based on societal norms that impact a woman's ability to do well and men's inability to recognize it. I can touch more on it if you'd like me to.

Statistically I don't know the exact number. The personal choices are social norms that we've imposed on women (e.g. women should stay home with the children, women are not as smart and therefore should not be going into STEM fields, a woman should be worrying about her family and not a job because that's meant for the man, etc.)
