Not sure if I understood everything correctly. But usually women want the men to make the first meaningful steps. You should work on being oblivious about signals. Well it it sounds to me like some sort of Trauma. I had an ex gf once who was behaving like that. I saw the pattern that she hurt the people closest to her because it had the smallest immediate negative feedback for her (in terms of verbal feedback and negative consequences for her life in a structural way) and also because she sabotaged herself. I'm not sure but I think it was something with her childhood. After therapy with a good psychologist it improved. So I'd say maybe go to a psychologist…
