My sergeant in the Marines had a foreskin that would not retract. He could pull it back manually, but with difficulty. He described getting an erection like putting the head of his dick in a vise. A military doctor suggested circumcision and after the procedure gave him some kind of ointment that reduced the sensitivity of his penis. He used the ointment for roughly a month then tapered off with no ill effects. It sounds like this poor kid was victimized by a bad doctor.
