They get a staggered normal serum testosterone level with a diurnal variation. It's not perfect but it's sufficient to cause puberty and keep a normal testosterone level. You can even measure pituitary hormone levels to ensure you're keeping up with what's required, if the FSH or LH goes up you know you're running a deficit and need to increase the dose. Hypogonadism is well understood and can be well managed. It's not a barrier to normal pubertal development or sexual function.

That's an interesting comment, and one I've had to reflect on. I'm not in it for the money - the UK doesn't pay nearly as well as the US does and if I were chasing coin I'd probably move to the states. I earn around $100k (although this is heavily taxed) whereas in the US the median salary is around $430k with lower rates of tax. Actually just to give the other side of the coin for a second - I'm an anaesthetist in the UK so, of course things may be different over here. I've never once been in a situation where somebody has anyone slagged off a patient when they've been asleep. Sometimes we comment on silly tattoos, question how on earth they've managed to get into an operating theatre without having a bath since admission or guess how many piercings they have but certainly no where near to the extent that this case says…

I don't think it was unnecessary. It gave her the opportunity to apologise, to show some remorse and contrition. If she'd done that he may have knocked it back down to 30 days. As it was she continued with the attitude and got herself into more bother.
