Every modern Western woman starts life as an evil child I'd say that every child is born evil but are then molded to become good by society, however it's sometimes hard to keep the eyes on the ball and mold someone when they also have a product of worth on them. If the doctor is a gyno or a procto or whatever position that adds some tension and some awkwardness when the patient is examined, he will try to joke with the patient to defuse the tension.
