The guy is a moron. You don’t escalate a conflict when the other person has a firearm. That’s just asking for someone to get shot in the heat of the moment. The gal is a moron also. You don’t pull a gun unless your life is at serious risk and you are ready to use it immediately. Brandishing a firearm is a class 1 misdemeanor (in Virginia at least - other states may vary) and can get you a year in prison.

One option for him is to get medical training through the military. If he went to the Uniform Service University of Health Sciences, he would get paid as an O-1 while in school. He would owe the military time after residency, but it basically takes the place of paying off school loans. This is what I did and I was pretty happy with the approach. A number of my classmates had children while there.

No - there's no hard rule. There's no hard rule to the vast majority of human interactions. It's theoretically possible that he had something come up or his phone ran out of electricity or any number of other things, but ultimately it doesn't matter, if he is interested in you, you will be a priority.
