It was initially used to describe the sadness/dissatisfaction one felt with their body/assigned gender. But further studies went on to show that the root cause of those feelings, most of the time, was the rejection and demonizing they experience from society at large. Basically...if people weren't complete pieces of shit towards trans folk...they wouldn't be as upset or as depressed as they are.
