You know that for sure when circumcission is the very first option considered. In fact, vast majority of phimosis can be treated with, in order of severity: stretching the skin over time, small incision, partial removal. Only the most extreme cases actually require full circumcission. *Source - I live in Europe, so only had small incision done, and there never have been any problems since.
