It really just depends on the shape of the hand. If it's cupped to force additional air through the eardrum then it will be more damaging. Also, damage to the cochlea is usually permanent. I've heard boxing both ears can lead to unconsciousness but I also know it's possible to permanently damage a single ear by boxing it solidly.
