In my previous comment I was just saying that at birth it's mainly superficial and unnecessary most of the time (unless there's a medical reason to do it right then and there). Later in life they are free to make that choice regarding their genitals if they choose to do so. Having that choice in what they do with their body is the theme of the comic posted.
