Really? Everything I'm seeing says that most do cover vasectomies, and some even cover reversals. Do you have a source saying that a majority do not? Insurance companies are about profits, and vasectomies are cheaper than tubas ligation, and MUCH cheaper than children.

What medical birth control is there for men, aside from vasectomies? And don't most insurance companies cover vasectomies? Expecting insurance companies to cover condoms is the same as expecting them to cover spermicidal jelly.
