There are many valid reasons why a provider may decide that bicillin+rocephin is more appropriate for a patient than azithromycin. When the shots are more appropriate, I personally would typically choose to give it as 2 shots, each mixed with a numbing agent, and advise the patient that they will have to endure 2 sticks that will be uncomfortable or 1 stick that will really hurt,and so it's going to be 2 sticks in 2 different muscles. The reason many providers choose the shot is not to be punitive. It's because the shot is one&done treatment but the course of pills is multiple doses, taken at home, where the patient may not take them correctly, at the right time or right food recommendation, may skip doses or may not finish them all. That could lead to drug resistant strains, and unchecked community spreading. If the provider gives the shot, then they can be more confident they have served the public health.

She could be one of the RNs that converts orders into Rx. When I still worked in pharmacy (lead tech manager during and for a short while after nursing school), we had quite a few private practice nurses who took on airs conflating calling in Rx, or faxing over authorizations on behalf of their OMDs, with doing the actual prescribing.
