I know this super late to the show, but you're stats are similar to mine and I had a ton of issue getting enough food. Best thing I found was a protein shake, like what you do, two scoops, 1/2 cup oats, 1 banana, 2 tbs peanut butter and whole milk. That right there is nearly 1k calories and all the good stuff you need. I throw in kale/spinach and/or raw eggs now and then if I need more. Tastes delicious and I can chug 2 a day if I really need a ton.

I think you've touched on the two main things - fear of not getting sex, and fear of dealing with angry outbursts. If you can become "okay" with these, it seems to be the linchpin that enables you to hold your frame.
