In Canada, it takes repeated visits to your doctor. I can't really see it being any different anywhere else. The doctor visits will establish a record of your mental health and, from what you're having to deal with, yours is very strained. Once you have the history set, get a medical letter clearing your leave. It usually takes a few months to get it all going. I've never done it myself, but I've seen it done a few times.
