If enough of a users videos are reported, they could do a preliminary investigation into the account to see what content they've been posting. In the end it really is left up to discretion however, so you're right in that there'd be no surefire way to fully tell, unless you (for example) find out the participants involved are pornstars, participating in other videos. Majority of the time people aren't willing to listen to a more drawn out and reasonable approach such as that. (and that's speaking in general) I've found over the years is that people these days are less open to having a conversation, and are more quick to place someone in a group or camp based on surface level interactions, unfortunately.
