It's more like the worst case scenario of this is that society leaves you to die. As in literally. As in you end homeless, and then you end up dead. This might seem a little like catastrophising, and it mostly is. This is the ultimate threat, things have to get pretty bad to get to that point, but the threat is always there. Subtly, in the background. Things have gotten better. Being seen as gender non-conforming is not the threat it once was, but it hasn't gone away either.

Death. Eventually. To not fill the social role of a man, is to be seen as surplus to requirements. People who might have supported you will instead, back away. Just because you did nothing wrong, does not mean the relationship has not fundimentally changed. You cannot expect things to just go back to normal after a scenario like that. See, the thing is, the frend in this question also has their own emotional and physical needs.
