I think a possuble unexplored answer is stated within the first few sentences. The first time you were going to have sex, you couldn't get it up. Regardless of how you play it off, that's going to hurt her self esteem and make her think she wasn't good enough for you. You can fix that by either telling her how much you think shes sexy and turns you on, or just enough foreplay so her brain isn't worried about what you think. Fail once more and you may never see the girl again at risk of her ego.

Im going through similar feelings after having and being a fixer in most of my relationships. It feels weird to have firm standards and turn someone down even though initially interested. I think you're over thinking it though. It feels terrible to know you've given up opportunities with people who wanted to be with you only to have them move on, but theres a silver lining. You're overthinking it. She likes you. This isn't some hook up where you're to be judged by your sexual prowess. It doesn't matter how good or bad you are because she likes you.
