This is the only advice anyone on this forum needs. Few people will take it or read it but coming from someone who went through a lot of unhappiness and loneliness, this is the only advice you should build a foundation on. I had the same problems you did. I beat them. You can do it. But make sure you have something specific you want to work on when you approach. Maybe you want to see the worst that can happen if you keep talking after you feel awkward, or if the girl starts to think you're sexually interested, or if onlookers think you're hitting on her, and so on. Those should be your goals, not "doing approaches". That's a bit too vague

In my experience focusing more on outer game made me rely on external validation. sure my inner game improved, but IMO when you're an afc with girls, you're usually also beta in life. I used to be streaky and i used to rely on getting (more like falling) into state. Better to tackle the problem from a holistic standpoint. I think one of the problems we have with approach anxiety is that the community pushes us to go into attraction immediately after opening. You don't have to do this. If there's a time constraint - that is, there's an impending interrupt - then simply befriend her. Run comfort stuff. You don't HAVE to attract and escalate immediately or even the same day you approach.
