Be playful about it (make something up), and then they'll want to know what really happened. Telling them about your transplant gives you a whole other facet, some more depth as a person. Just don't focus on your current medical problems because that can get awkward, and generally be a downer. What type of transplant did you have? The scar sounds too huge for a kidney or liver.
