I agree we can have different opinions, and I enjoy this type of debate! I disagree that orgasming is more intimate than shitting. My husband has seen me orgasm; he hasn't seen me shit. It's a silly point for me to get hung up on, I know. I do feel like we're kind of in a suffering olympics at the moment, a 'who has it worse with medical procedures'. I hate to gatekeep, but you haven't had the test yourself yet. Please don't be scared of them, but yes, it is a painful, scary, and embarrassing procedure. However, it's better than the alternative, so I get it done. I've never been offered a different speculum, most clinics are still one size fits all. If you're super nervous you can insert the speculum yourself, and that's about it. I personally find shitting myself while someone's head is down there more embarrassing than orgasming, but then that's just me. Depends on which part of the pap smear you're talking about. When they take the scraping? If they're experienced, feels like a pinch. If not, it feels like fingernails scraping inside of you. The speculum they use to open you up is the worst part. They only recently started using plastic ones, and different sized ones. Of course, if you're one of many women with vaginismus or a history of sexual assaults, pap smears are a lot worse.
