A lot (but not all) of our current sexual behaviour are evolutionary artefacts from our ancestors who had higher fitness due to, for example, men being more promiscuous and women being more choosy. A male making a poor choice of mate only loses time and sperm. A female loses her entire term of pregnancy and the massive resources it costs. There wasn't a grey area here, she didn't want unprotected sex. Any part of sex that you explicitly said you DIDN'T want to do, then is performed anyway, is sexual assault.
