Check out the books "Feeling Good" and "Intimate Connections" by Dr. David Burns. He's a renowned cognitive behavior therapist with some great ideas and exercises in these books. Feeling Good is mostly about treating depression and anxiety but he has a couple of chapters in it about improving self-esteem and confidence.
