We were talking about the person. Surgery doesn't change the person, the person is already that, in fact surgery is not even necessary all the time - she didn't use to be a man, this is the first mistake: she was assigned so, mistakes happen, because sex is a complicated thing, it's a double bell curve, with a lot of factors that don't fit the majority.
