In the skin there are a bunch of different cell types growing and by placing skin in a liquid containing the specific nutrients for fibroblasts and not other cells you can get a pure fibroblast cell line. Why they thought that this would be a good idea to market as a foreskin skin cream I have no idea. Then the scientist gives his cell line to ATCC who then makes tons of cells then freezes them to give to other people (mainly scientists but also for profit companies). These cells last decades frozen and help with scientific research. Foreskins are not for sale and at most a few are needed a century to keep this process up. The reason they use foreskin is that its the easiest way to find a skin sample donor. The alternative would be to try to find donors who are willing to get a piece of healthy skin removed. Even if the growth factors in this cream originated form a foreskin that one foreskin also contributed cells to cancer and other medical research. At most they would just culture the cells and use them to produce their growth factors. However most likely they do not use anything directly from the foreskin cells and instead just add the growth factors. The growth factors that produce collagen (what they claim is special) are well known and you can buy online. I'm not sure how the online vendors produce them though.
