I wish I could help you, but I too know this feeling, and minus the reconstructive surgery, your life sounds eerily similar to mine. The only advice I've been given is "You attract those sorts of people because you're more confident around them. You're not thinking in terms of dating, so your true self comes out a little more and that's what's attractive." I never thought myself to act any different around women I was interested in, but it makes a lot of sense really.

Heh, now you're asking for details. That's the thing though, while there's plenty of material out there, reading it and memorizing it doesn't do a whole lot of good. You just have to be confident, approach girls readily, and learn from your mistakes. As someone else (whose name slips my mind at the moment), "You don't expect to learn to play the piano by just sitting at it, right?"

There's a quote from Barney Stinson that I feel is relevant: "Remember, it’s not about scoring. It’s about believing you can do it, even though you probably can’t." In otherwords, relating it to your story means that it's only bad if you perceive it as bad. If it makes your confidence falter, then do away with it. Otherwise, just do what you want because you're that awesome.

One-itis is a terrible thing like that, but I've been there so I can sympathize. Just don't make it awkward and it won't be. A simple "Hey, we should go out on a date sometime" should suffice really. If she makes it awkward, that's her fault, not yours.

Each situation is a little different, so it's tough to give a lot of advice. Best thing I can say is be confident and make happen what you want to happen. What's the worst that could happen? She rejects you? Move on to the next one.

Fix your self image, easier said than done right, but you can do it. Are you in college? Odds are that college has a gym you have access to (or there's a local gym that will give you pretty steep discounts if not), start taking advantage of that. Hop over to /r/Fitness for tips if you feel like you need them. Find a clothing style that has sex appeal and that you enjoy. You don't mention what you like at all in this post, so I can't really point you in any particular direction, but general rules…
