You don't, because it is weird, makes conversation awkward, and might make her feel trapped. Sit across, and if you want to escalate, do it by touching her hands across the table, or with good-old-fashioned footsie.

I'm not a woman, but this doesn't sound normal to me. Either you're doing some very rough stuff in the bedroom, and should calm things down and find a better tempo for both of you, or you really should see a doctor. If yours isn't helpful and understanding, see another doctor. What you're describing sounds like misery, and I would be horrified if I found out someone was going through torture to make me happy. And I wouldn't allow it as soon as I found out.

Tl;dr: women want men to be both thoughtfully respectful and viscerally passionate, and that's a hard balance to strike.
