It should be normal. I live in the Southern US and it's not normal for women to get the door for men. It does happen but it's not the norm by any measure. And to reiterate: she got the door. She wasn't leaving and then just held it. She actively sought to help in a situation where I could have probably just gone on through with a little fight. I bring it up only to temper the misogynists. :)

It's not that simple at all. That is but one aspect of life, and it has nothing to do with how people treat each other (or shouldn't). Just so long as you realize that what you're trying to call equality is really adherence to gender roles, sure (man: big, strong, make money; woman: cook, clean, good mommy, spread on demand).
