It's all about context. If a medical doctor calls themselves a doctor at a physics conference they're going to get the side eye from everyone in the room as well. A lot of academic doctors look down on medical doctors who didn't do any original research or some kind of dissertation. I've heard PhDs call MDs glorified technicians. It's stupid gatekeeping either way, but medical doctors and people who never went to university are the only people who think medical doctors are the ultimate doctors.

That's really only because you work in the medical field. The misconception that "doctor" means medical doctor is so prevalent amongst the general public that it's reasonable to assume you'd be confusing people if you called yourself a doctor in a medical setting. That being said, you're still absolutely a doctor even if you don't have an MD, and if you wanted to be referred to as doctor in a setting where it wouldn't have the possibility of causing harm, then that's your right. True but it's commonly understood by the general public that saying your a doctor means your an M.D. There are lots of things "commonly understood by the general public" that are nonsensical and wrong. Someone with a doctorate is a doctor and that's how you should refer to them if they request it. People with PhDs we're doctors before medical doctors existed and were allowed to use the term doctor as well. Doctor literally means teacher, which applies more to academic doctors than medical doctor…

Studies show that the best AA or 12 step groups work about as well as cognitive behavioral therapy or motivational enhancement therapy, so it's certainly not AA or bust.
