I would argue that (4) works slightly differently. I usually give her my number, sometimes you just miss each other or one of you is accidentally late due to unforeseen circumstances, it helps you find each other. Happened to me before, no big deal.

By asking her out, in the second or third message. No need to overcomplicate things, be direct about it, just ask her if she would be up for a coffee or something. No one likes the awkward penpal thing. I'm with David Wygant on this one. You just say "what a lucky guy he is". If she really has one, she will reevaluate whether he really deserves her and you will come across as a confident and nice person, someone with dignity. Either way, you can just ignore the (perhaps only hypothetical) boyfriend and proceed.
