Perfect use means lab settings. Even then 2% failure means it can fail Regular use means regular use, not idiot use. Even if you apply and use perfectly in your mind, it could have been stored improperly at the store or while shipping and overheated and cooled too much or something. It’s far more likely that the condom just failed than the hookup waiting with a pin for OP to go to the bathroom. Plus he doesn’t even know if he is the father yet and she hasn’t even said he might be.
