Don't build it up too much. Go out, have some drinks, click, be physical with your girl, and calibrate/escalate carefully and respectfully. Let your girl facilitate or her friend participate on her own.

The only question you've asked here is regarding payment. Expectations will be that you offer to pay. Whether or not you behave in this manner is entirely dependent on your personality and what you're looking for. I personally offer to pay if I enjoy myself with a hope that they'll offer as well. If they do then I go on a second date. If they don't then I don't (unless they're extremely appreciative). If I don't have a good time then I'll just ask for a split bill.

I've worked through crippling anxiety as well. I got over it by starting small. 1 conversation with a stranger per day 2 conversations with a stranger per day ... 5 conversations with a stranger per day 1 conversation with a cute girl per day ... 5 conversations with a cute girl per day 1 approach with intent to set up a date ... 5 Incrementally expose yourself to riskier endeavors that show you how much more potent a single success is than a dozen failures.
