It was named from a collection of features which were later (about 100 years later) found to have a common cause. Big tongue, epicanthic folds, heart defects, single palmar crease, low IQ. Loads of other stuff. We still don't actually know why a second active chromosome 21 causes these features, or the ones that were added later into the syndrome - early dementia, cancer risk.
