It's another manifestation of the hyper/hypoagency dichotomy. Men act and bear full responsibility for their fate; women have no agency and are simply rudderless ships bounced around by external forces. The fact that men and women both are beings with agency who're nevertheless heavily restricted by broader norms never enters into the picture, and that's the core failing of popular feminism.

1) In some way, society taking trans men seriously is a sign that society doesn't think trans men are really men, because if they did, they wouldn't take them seriously. So in a roundabout way, it's an insult toward trans men, by granting them privileges usually reserved for women. That can't be especially heartening for them (or, at least, it's a mixed blessing). 2) It is what it is. We can either be upset about social hypocrisy, or embrace it as a valuable tool to be heard. That's really the thing that gets me. There are consistent sets of principles, and although you can certainly say one or another leads to a better life (everyone should always assume the worst of their spouse vs everyone should strive for a relationship defined by love and trust), at least they're consistent and there's room for genuine discussion. Here the book is just sardonically referencing the common phrase; it's entirely about how outdated gender norms lead governments and NGOs to ignore men and boys who are in war zones and who are in desperate need for help.
