A disease, such as a tumor or virus, is something that attacks its host. I would consider a brain with anxiety, depression, or addiction that's attacking its host to fall somewhere near the same category. It's not a really a choice at the point where you finally subdue to the exhaustion from it. A lifelong smoker getting cancer, or someone who drinks 12 Coke's a day getting diabetes is more of a choice than a mental illness.

It's mostly just the way people interact. If they're smiling and joking around you can just see they're in love. If it's awkward you can tell it's forced. Parents are a telltale sign when they're too needy and insistent on the occasion being spectacular. It really just comes down to if the couple actually wants to get married or if they think it's a responsibility. You can tell the difference.

I've been a caterer for weddings that cost this much. You can tell which ones will end in a year. Either way it makes no sense to fork over that much cash. But they gave me a paycheck and I gave them a thumbs up. I will most certainly not pay for an "official" roommate.
