Based on number of diagnoses. NPD has one of the lowest diagnostic percentage. How many narcissists do we all encounter in everyday life, and think about how many are diagnosed. Probably 0.00001% of them. NPD is one of the most therapy-averse conditions, so you won't even see them in a psychiatrist's office, and it's one of the only conditions that can actively hide itself. The real answer is no one knows.
