it was my understanding that to get Plan B involved a physical as well as questionnaire. Perhaps that is just the free Plan B from clinics. I assumed "Birth Control Failed" was a euphemism for "I went bareback last night" which would be the reason given for seeking Plan B. Ironically, I don't know for a fact, but I believe the questionnaire was meant to elicit whether or not the woman was raped and was hiding or embarrassed to say. I doubt they do it anymore.
