No, the APA recommends 2 years of breastfeeding. The 6 months is for exclusive breastfeeding (i.e. no solid foods.) 13 months is perfectly fine to still be breastfeeding and "far too long" is a subjective judgment; the ideal weaning age will be different for every child and family. But I do agree they need to sleep train this baby and start cutting back on night nursing so they can get an adult life back. A 13 month old is capable of sleeping alone.

Wow, I never even considered dressing down for doctor appointments. To me that's someone who could potentially see me naked as part of their job duties, so if I felt any type of weird vibe I just wouldn't visit that person again. It's nerve-wracking enough dealing with medical issues without adding any extra concerns. I prefer female doctors in general anyway, maybe that's part of why.
