A fat man is as manly as he acts, and a fat woman is a "girly" (the fact that you use this makes me think that you want girls and not women as a partner which could be fucked up if you're not into consensual relationships)/feminine as she acts. (I'll assume) He doesn't just mean physically, but also mentally and spiritually healthy.
