Is it less unhealthy simply because somebody else is doing it?The answer is that 'self-harm' is a vague term that could be applied to a lot of things outside of responsible BDSM. As far as being a detriment to mental health, I would think it would imply extreme stuff like lacerations or Munchausen syndrome and in extreme self harm, I'd argue that the acts don't actually harm one's mental health but instead are a symptom of an existing mental health problem and maybe help sustain rather than create it.
