Unenlightened woman here: It seems reasonable to me that a girl might ask that question as a gauge of the guy's interest. I'm not saying that it's right or that I would ever do it, but in this particular situation with a girl you know, a more friendly rejection line like the one you didn't use ("Sure, what are you buying me?") or something like the thumb wrestling one might have been more appropriate. It sounds like she may have been flirting with you and was disappointed that you didn't recipro…

Yeah, it's pretty difficult to tell what the real story is here. I think the fertility clinic would have known if a single woman was forging the signature of a man. Plus, they had his bloodwork on file. That's pretty damning. What, was he trusting her to discard of his used needles as well as his condoms? And in several months of dating her, he never noticed that she was being pretty fucking weird about his spunk?

I was a kid who really enjoyed playing with plastic animals, and had a huge collection. I'm not sure what this one dad is on about, but I can tell you that sexual dimorphism was commonplace in my figurines. Plenty of them had nipples, and plenty of them had penises. The better the quality, the more likely it was that it would be anatomically correct, including secondary sexual characteristics. My parents explained that penis = boy and still didn't give me the talk until I was in high school.

I can't speak for all women, but as soon as a man mentions my physical appearance I immediately write him off. You're doing something wrong here, so why not use a scientific approach to narrow it down? Try other openers to see how they work.
