Gender identity is the usually inflexible notion within an individual what primary and secondary sexual characteristics they are most comfortable with and which gender they are most comfortable being associated with. And to answer your question: Because of a fun little thing called "gender dysphoria". Back when I was presenting as male, I was depressed to the point that I could not function. 3	Transexuals vs mental illiness. Except that transgender people don't have a disconnect from reality. It doesn't even fit the definition of a delusion: certainty, incorrigibility, impossibility. Even if you want to argue the last two, there's no way you can argue the first.
