Virgin = a person who has never had sexual intercourse. Of course it is a sexual context. And according to my quick google searches, both covers didn't "garner massive outrage". Even after all those decades, the albums are still sold with those covers.

Hang on. The album depicting a naked 10 year old girl is called "virgin killer". That's like if nevermind was actually called pedophilia. The name of the album in addition to the naked girl has massive sexual connotations.
